How do I create a fillable PDF and Save to webserver?

i have pdf form setup , have added fields.  want save copy on webserver via https person can login , view via https.  form contains sensitive information.  can save mysql database if needed, save entire pdf fields filled , name field name idtification lastnamedate.pdf.  best way save pdf docs?  should email them instead?  pdf employment application need secure spying eyes.

to put on server, copy there , add link form. not sure if there special https servers, doubt it. talk sensitive information , such. eliminates e-mail not secure @ all. if form need filled out , submitted, should submit secure server script same reason. application forms going used more 500 times, should not activate reader rights, collect data data submission in fdf or sfdf form (using acroforms) or in xml form (using designer forms). each of these data formats can parsed put data in data base or imported form in acrobat.
