Email capture help

i building style guide , 1 of features needs able capture email address updates on product.

 

what looking have user input email , have saved notation on product being requested. list or email sent address subject line being product name. assume i'll doing of in flash builder, i'm not sure need before (i have text input , submit button sorted, don't know tell them do). appreciated.
